I have seen this on multiple machines

I have removed and downgraded to 8.1.7.0. I went back to 8.1.6.3..

My guess on the conflict is Adding a second Playback account

Additional Playback account still exists with 8.1.6.3 DB errors are now gone

I use addons, after hours and hours on Sunday. I finally discovered some new test devices had this setting on. You never get prompted to close tabs. You end up with a modal dialog box you never can see to clear.
